Our home care services

We care for you with comfort, dignity and privacy in your own home

Frail care services

Our frail care services are for people who are elderly and frail, or suffering from Alzheimer’s disease, dementia and other chronic diseases and disabilities.

These services are provided by registered nurses, social workers, companions and / or carers.

Home infusion services

Our home infusion services cater for people suffering from infections (pneumonia, cellulitis, Cystic Fibrosis) or other conditions, and who prefer to receive intravenous antibiotics, immunoglobulins and other infusions at home rather than be admitted to hospital or rather than remain a prolonged time in hospital.

These services are provided by registered nurses.

Home wound care services

Our home wound care services are for people suffering from wounds after surgery or trauma, or from ulcers (diabetic, pressure or ischaemic) and who prefer to receive wound dressings at home rather than be admitted to hospital or remain a prolonged time in hospital.

These services are provided by registered nurses.

Post-natal care services

Our post-natal care services caters for women who prefer to leave hospital shortly after childbirth, and receive post-natal care in their homes by registered midwives and carers.

Palliative care services

Palliative care services for those people suffering from advanced life limiting conditions, including

  • cancer
  • heart failure
  • lung disease
  • liver failure
  • kidney failure
  • multi organ failure
  • AIDS,

who prefer to spend their remaining time with their families in their own homes rather than in hospital or hospice.

These services are provided by registered nurses, carers, physiotherapists, counsellors and social workers.

We are contracted to major medical schemes

Physical rehabilitation services

The physical rehabilitation services cater for patients recovering from major illnesses, strokes, motor vehicle accidents, hip replacements and knee replacements, who prefer receiving physical rehabilitation back to wellness in the comfort of their homes rather than hospitals, step-down facilities or rehabilitation hospitals.

These services are provided by registered nurses, carers, physiotherapists, occupational therapists, speech therapists and social workers.

Chronic incapacity services

The chronic incapacity services cater for patients with severe incapacity, including severe brain injuries, severe multiple trauma injuries, and patients on ventilators, where families and friends prefer care to be provided at home rather than the patient remain for months to years in a hospital or step-down facility.

These services are provided by a doctor-led team of registered nurses, carers, physiotherapists, occupational therapists, speech therapists and social workers.

Mental health and substance abuse rehabilitation services

Our mental health and substance abuse rehabilitation services are aimed at people suffering from severe mental health disorders including:

  • Bipolar
  • Dual diagnoses
  • Psychosis

Substance abuse (drug and alcohol additions) who prefer care at home rather than remain in a psychiatric home.

These services are provided by a doctor-led team of registered nurses, occupational therapists and social workers.

Home Care in lieu of prolonged acute hospitalisation 

SA Home Care is able to reduce prolonged acute hospitalisations via the aforementioned services, most particularly:

  • Palliative care / Cancer services
  • Physical Rehabilitation services
  • Chronic incapacity services
